By submitting this Application, you agree that all the information you have provided is true and correct. Providing any untruthful answers or failure to comply with the requirements of this Application can result in the forfeiture of the adoption.

We will require the successful applicant to have the animal on a trial basis, with a length of time determined by 9 Lives Cat Rescue, and a possible inspection of the premises on which the animal is going to be kept. If after the trial period has ended, it is determined that you and the animal are suitable for each other, Adoption will be made permanent with the signing of an Adoption Contract.
